We provide support to people who are misusing alcohol and who wish to make changes to their drinking. Our staff work in partnership with On TRAK to deliver structured support and interventions to people who are unable to come into the On TRAK service because of physical illness, disability or poor mental health. We visit people in their own homes.
Our team will deliver the On TRAK interventions to people in their home. The length of this support may be between 3 – 6 months. This depends on the help needed (for example reduction, detoxification, harm minimisation advice) and the complexity of the individual’s needs (for example someone may find shorter appointments easier to cope with due to anxiety or illness).
The emphasis of the service is to help people to make changes to their drinking. It is therefore important that at the point of referral, clients are ready and motivated to follow the goals agreed with their worker and to take the steps to make changes.
